New Jersey Disability Parking Placards: Who Qualifies and How to Apply
When walking across a parking lot causes pain, shortness of breath, or exhaustion, getting to an appointment or picking up groceries can become a challenge. A disability parking placard can help eligible New Jersey residents access designated parking spaces closer to their destination. If you are searching for a New Jersey handicap parking permit, disability parking placard, or wheelchair symbol license plates, this guide explains the eligibility requirements, official forms, and application process, including in-person and mail submission options.
Who qualifies for a disability parking placard in New Jersey?
Eligibility depends on specific state requirements. Qualifying circumstances include:
- Loss of use of one or more limbs because of paralysis, amputation, or another permanent disability.
- A severe, permanent disability requiring a walking aid or another person’s assistance.
- Lung disease with an FEV1 below one liter or resting arterial oxygen tension below 60 mmHg on room air, or use of portable oxygen.
- A heart condition with functional limitations classified as American Heart Association Class III or IV.
- Severe, permanent walking limitations from an arthritic, neurological, or orthopedic condition, or inability to walk 200 feet without resting.
- Permanent vision impairment in both eyes certified by the New Jersey Commission for the Blind, for a placard only.
A qualified medical practitioner must certify eligibility. Having a diagnosis alone does not automatically establish that you meet the criteria.
The current application also includes a pathway for veterans certified by the U.S. Department of Veterans Affairs as permanently 100% disabled.
Which New Jersey disability parking form do you need?
Use the official state forms:
- Form SP-41: Application for Disability License Plates and/or a Placard for a permanent placard, wheelchair symbol license plates, or both.
- Form SP-41A: Application Instruction Checklist to review required documents and avoid missing information.
- Form SP-68: Application for a Temporary Placard for a qualifying temporary mobility limitation.
Start with the appropriate form and read its instructions before arranging your medical evaluation.
How to apply for a permanent disability parking placard
1. Complete your portion of Form SP-41.
Select the application type and whether you are requesting a placard, license plates, or both. Complete the applicable sections and signatures.
2. Obtain the required medical certification.
Your practitioner must evaluate whether you meet the eligibility requirements and complete the certification section. SP-41 also requires an attached certification statement on a prescription or the practitioner’s official letterhead. Submit the application within 60 days of certification.
3. Gather your supporting documents.
Follow the SP-41A checklist. Use original medical certification documents, and include a legible copy of your current vehicle registration if applying for wheelchair symbol plates. Complete all applicable signatures before submitting.
4. Choose in-person or mail submission.
For permanent placards and wheelchair symbol plates, New Jersey offers both options.
Option 1: Submit your application in person
Bring your completed application, medical certification, identification, and required supporting documents to an NJ MVC agency.
Review the official MVC application instructions and agency information before visiting.
Option 2: Submit your application by mail
Mail your completed application and required documents to:
New Jersey Motor Vehicle Commission
Special Plate Unit
225 East State Street
PO Box 015
Trenton, NJ 08666-0015
You can choose either an MVC agency visit or mail submission for your permanent application.
How to apply for a temporary disability parking placard
Temporary placards are intended for qualifying temporary mobility limitations. These may include temporary loss of limb use, needing an assistive device to walk, or other temporary restrictions on mobility. Medical certification is required.
To apply:
- Complete Form SP-68.
- Have a qualified medical practitioner complete the certification.
- Submit the application in person to your municipal police department with a $4 check or money order payable to NJMVC.
Temporary applications go to the municipal police department, rather than an MVC agency. The department issues the placard after approval by the municipal Chief of Police.
A temporary placard is valid for up to six months. One additional period of up to six months is permitted with medical recertification.
How much does a New Jersey disability parking placard cost?
The MVC lists no application fee for a permanent disability placard or wheelchair symbol plates. Temporary placards cost $4. Medical evaluation fees are separate from government application fees. Normal vehicle registration costs still apply to vehicles with wheelchair symbol plates.
Do permanent disability placards expire?
Yes. A permanent disability does not mean the placard lasts indefinitely.
New Jersey requires medical recertification every three years. Renewal involves an application, supporting documentation, and a new medical certification dated within 60 days of submission.
You can renew a permanent placard in person at an MVC agency or by mail. Wheelchair symbol plates also require annual registration renewal.
Can you use a placard if someone else drives?
Yes. A qualifying person may use a placard as a passenger. Carry the accompanying Person with a Disability Identification Card when using the parking privilege. The privilege belongs to the eligible person. A family member or caregiver cannot use it for their own parking when the eligible person is absent.
Can telehealth help with a disability parking evaluation?
A telehealth visit can be a starting point for discussing your mobility concerns, medical history, and documentation needs. Whether a clinician can complete certification remotely depends on whether an adequate examination and assessment are possible. Both application forms require the certifying practitioner to confirm that they personally examined the applicant. Additional records, testing, or an in-person examination may be necessary. Certification should only be completed when the practitioner can support that the eligibility criteria are met. A medical evaluation does not guarantee certification or a placard. The clinician’s role is to assess and document eligibility; the MVC or municipal police department handles issuance.
